Effective management of change (MOC) is vital for safe, reliable operations. The U.S. Chemical Safety and Hazard Investigations Board (CSB) and the Center for Chemical Process Safety (CCPS) indicate that a vast number of chemical incidents are a result of poor implementation of process safety management, and in part also due to issues with the operator’s MOC process.
